OK, we have tracked down the issue... it's relating to a custom variable we set called "Hide from list".
Easier than reverting to DRAFT mode, but we needed to check if value set when getting query of items in a list... pagination is counting the hidden items, thus only 4 on page 1, because 4 items are hidden in first 8... we edited but needed to SAVE previously edited collection items for them to show up.
