perch page has no regions - but is using a template

I have some odd behavior with one page. All the pages on the site use the same page template. The code on the about page shows the link to perch and the correct template. The content is on the page, the page is live. But, when I go into the CMS to alter the page there are no regions on the page. However, the other pages built with the same template have the regions available. The same behavior is happening on my local server, my remote dev server and the live server.

I tried deleting the page and creating it again using the same file name and the page is automatically filled in with the correct content. But, there are no regions attached to the page.

If I create a brand new page using the same page template but a new file name the regions show up. How did this happen? Odd behavior indeed.
