Displaying custom fields on the cms for comment app

Hi Drew,

I finally got the comment app to work. Just few minor issues, I can't seem to display the image uploaded by the user in the front end and also the added custom fields that I made with the form is also not visible for the admin to view. Is there also a way to have the default status of the comment to pending?
