Perch Forms Not Showing Detail

For some reason my Perch Form entries are not showing any 'detail' in Listing Responses.

The form is working fine and all of the information is being collected correctly when looking at each response individually.

Any ideas?

If you add the label attribute to your perch:input tags, you'll see them in the listing. If you click through to the detail page, you should see all the fields regardless.
