MailChimp - opt-in to list when completing another Perch Form

It says that it's possible to opt-in to list when completing another Perch Form in the docs for the mailchimp app but I cannot find an explanation of how to do this in the documentation.

Is it possible to display the opt-in as a checkbox?

Can you tell me how or post a link to the relevant information.
